Your ID should display your address and photo. Driving licenses are the best option. The billing and shipping address must match that shown on your ID.
If you do not have a driving license, then please upload another form of ID such as a passport. Please also upload another form of ID to verify your address. This can be a utility bill (water, electricity, gas) or a council tax bill.
